At any point, in this procedure, did you actually correctly eject the reader... I mean, do you normally connect a hard disk, write to it, switch it off and disconnect? No, you follow the normal eject procedures... now with an eReader, you either eject from Reader program, safe removal in tray,right-click or via explorer then disconnect from laptop and wait for eReader to finish its housekeeping before turning off...
