This specific issue seems to be caused by a bug in FlightCrew. I assumed you added the "N/A" part of the message, obscuring the name of the file (for whatever reason). But if FC is adding this, then that's a bug; it needs to list a filename. A different user has reported this issue as well with a test case.
I'll probably take a look at the problem tomorrow, I currently don't have the time.
