What I do is the following....
I don’t worry about the .CSS file at all.
1. Open the EPUB file with Winzip
2. Look for the page-template.xpgt file
3. Delete page-template.xpgt from within Winzip
4. Remove DRM
5. Add into Calibre
6. Convert using 0.0 pt margins set in the Page Setup options
7. I like page justification too
