After locking the zoom level, press Home button to quit. Next time when you select the same book, the zoom-locked screen will be shown. This is maintained per book. (P.51 of User Guide)

Press OPTIONS button, select 'Sort' and then 'by File Name'. You will see the file name right below the book title.

I think Sony did this on purpose. Otherwise, the same book copied later can overwrite all the bookmarks and annotations you have stored within a book.
