I've been meaning to give a second update to my problem for a while, but have been too lazy and/or forgetful.

I've discovered a system that works for me, and the formatting issues are much easier to resolve initially than before.

The new steps I go through:
  1. Strip DRM from epub (if necessary).
  2. Calibre the epub to fb2.
  3. Using a text editor (vi; Is there any other choice?), use global search and replace--with confirmation--to replace <strong> with <h2>, and </strong> with </h2>, in the fb2 file.
  4. Load fb2 file into CoolReader and use the internal (edited) fb2.css file. (I use the jotas firmware; it should work fine in Buggins or LBook firmware.)
  5. Enjoy reading the book!
That said, I still occasionally have problems, especially with Millennium. Sometimes I have to go so far as to shutdown and restart the PPro, and reset the clock, but so far it has always been fixable. I also sometimes use the Buggins cache clearing program, which helps as well.
