Is the endnote reference not a hyperlink? That's the normal way they are implemented in an eBook. It's a rather poorly-created eBook if there's no way to get to the note from the text.
Nope. And in the actual book they are footnotes.

How would you use the hyperlink in a Kobo anyhow?
