At Fictionwise, Kobobooks and B&N you can use any valid USA address as billing address of your non-USA credit card (for B&N you will also have to use a VPN like Hotspot Shield or alwaysVPN).

That is how I have been doing it, I think the only thing they check is that the Zip code is valid. I am, however, looking ahead and thinking about what to do when things get tougher. I have read that Entropay doesn't work in many places because it is UK based. Besides, 5% is a lot. On top of the credit card charges you have to pay to fill up your account.
