You don't need an US credit card.
Any valid credit card will do (I have a German one).
You can have numerous accounts respectively numerous residencies per account. You can switch back and forth between accounts. For example, Amazon has sent my Kindle DX Graphite to my German home address, although they see my US address on some of my orders. Since the introduction of the international versions of Kindle, they don't mind.
You need to register your Kindle on the Amazon website in your account ("manage my Kindle"). Your ebooks will be DRMed (copy protected) based on the serial number of your reader you give in that account. You have to register every single reader. Online units like iPhone do this on their own, with others you have to do it manually.

You don't need any gift cards. You can use your home credit card, even if you define an US account.
