Assuming it's not DRM'd, you could try doing a conversion using calibre. I've found that's sorted out a few problems in the past.

I can't recall for certain off the top of my head, but I think you can use calibre to do a "conversion" from epub to epub, which would eliminate any problems arising from converting to and back from an intermediate format.

Alternatively, you could try editing the file using Sigil - or at least using it to see if there's anything obviously wrong on page 3 or 4.
