It is strange though that the links in the chapters refer to the Index ! ... It should be the inverse. The links in the Index should be redirecting the reader to the corresponding page in the book.

A tool called PDF Workshop...

A tool called PDF Workshop (http://www.PDFWorkshop.com) merges the PDF files and merges their bookmarks too at the same time + updates the bookmarks pages numbers. I think it may do the job.

But...
