Troubleshooting TOC broken on new downloads.

Lately I've noticed that books I've formatted with a working TOC do not have a working TOC after they're uploaded to the Kindle store. (I don't mean just a linked TOC in the body of the book, but the one that comes up in the Kindle menu.)

When I format a book, I always download a sample from Amazon to make sure everything went through ok. The books had a working TOC before they were uploaded, but it's missing from the downloaded sample.

I also tried another experiment. I found a book I had purchased from Amazon awhile ago that has a working TOC. I downloaded a sample of the same book. No TOC.

Did Amazon break something?
