Reset Sony Device Interface plugin?

I recently modified Calibre's Sony Device Interface plugin but now I want to restore it back to its default values. Unfortunately I didn't not note the default values (Save template, etc) before I modified them. Uninstalling and reinstalling Calibre has no effect.

Does anyone know how to "reset" the plugin back to its default values? If not, then could you provide me a printscreen of the plugin's values?

I don't own a Sony so have never been into the plugin screen for this device before to change anything - attached is what it shows on mine.
