Adding forms to a Kindle book

Hi all,

This is my first question to the forum. I've searched high and low for an answer to this and I already suspect that the answer will be no. Is there a way of including a form (or a number of forms) in a Kindle book?

A colleague and I are working together on converting a personal development book into a Kindle book. The print version includes table/forms that allow the reader to compile lists of goals/desires etc. and we would like to include a similar feature in the Kindle version, if at all possible.

Any suggestions would be gratefully received, even if they are for workarounds.
