Generating Table of Contents

I am attemting to convert a Word file to epub using Calibre. I have marked all of the "Chapters Headings" as Heading 1 and saved the file as html.

Using Calibre, I generated a table of contents and converted to epub. The first two chapters are always missing from my Table of Contents. They are highlighted as Heading 1 in the original file.
