PDF to Epub conversion problem

When I try to convert a pdf to epub it loses all the formatting.
For instance, I'll convert a pdf of a 'Dummies' book and in the epub version after conversion the 'Hot Tips' or any other side tip in the book is thrown out and sometimes mixed into the regular bulk of the book.
