Could not convert 1 of 1 books

I get the following message when I click on "Convert Books"

"Could not convert 1 of 1 books, because no suitable source format was found".

This happens to every book that I have previously converted.

Calibre version 0.9.4

Calibre remembers the previous settings.
Did you delete the original (conversions) format?
Worse, did you delete it using the OS and not Calibre delete (format)?

As far as I know, I didn't delete any formats deliberately. How would I check that?

Run the Library Maintenance tool, Both parts. (Right click on the Library Icon)
