g cover image without losing first page of text

I convert from pdf often and sometimes there is no cover. The problem that Im having is that when I choose to download or change the cover it will replace the first page instead of creating a new page for the cover when there is text on the "existing cover". Is there a way to get a cover without losing the firs page of text in this type of situation??
