Small html/css bug
When converting html to lrf, and there is an empty <title></title> in the html header, calibre won't process the style sheet. It's a little annoying, because some programs will insert <title></title> without asking.

I doubt that has anything to do with the empty title element. Much more likely cause is the uppercase selectors like P and TD in the css.

Try it. Replace <title></title>, with <title>xy</title>, and convert it to lrf. Now watch the changed behaviour of the horizontal lines.

I did try it, but with epub output. Changing the selectors to lower case caused the css to be applied.
