It is very uncommon, Android apps are expected to work for any form factor. I only know of one app vendor that has a Phone and Tablet version, it is QuickOffice. They have a Pro version and a HD version. I have the Pro version and it works fine (scales up) for a phone, 7", and tablet. However the HD version only works on 10" tablets and has a lot more features. The UI was written for full doc writing/editing, which is probably why Google bought t hem out.

In iTunes, or on the iTunes website, any app with a little + (plus) icon supports both iPhone and iPad layouts in the one app.

Edit: Bluefire shows an example of the plus icon. The screenshot section shows both iPhone and iPad screenshots.
Thanks good to know the iPad and iPhone appstore on the iPad threw me off. In thee Google Play store and Amazon its just one store apps are assumed to work on both. If they don't they will not show up on the stores.
