Lodge, MD, is free in the Kindle store, under a mislabeled entry (that claims to be The Heart Foundation Book). I downloaded it and it apears to be complete, with 4463 locations and a full 21 chapters and appendices. There's no description (and no author) on the page where the book is free (and the title is wrong, but the cover is correct), so I lifted this one from a second Kindle edition, which is $8.85 and roughly the same file size).

Glad to hear it. It looked like an interesting read, sort of a conversation between the two authors (what of it I glanced thru, as I was checking to see if it was the entire book).
