Book chapters/story line
In a book, new story lines most of the time start at a new page. In my ereader, a kobo glo, it shows on the same page with a few blanc lines. Si it does not see the difference. It there a setting for that or do all ereaders do the same?

I wouldn't rely on any HTML code forcing a page break. A new XHTML file will, on readers that take notice. I believe the Kobo reader may be eccentric in this respect.

The Kobo readers will go to the top of the screen for a new file. Any time I have seen a new chapter just continue on from the old, it is in the design of the ePub.
