Epub Errors from Apple SOS!!

Okay so this is my first time dealing with Apple and their ridiculous specifications for the epub files they accept. I uploaded an entire batch of ebooks to Apple and got back just pages of errors. I think I've figured most of them out but this one I can't seem to crack:

9781621374008_EPUB.epub error ePub check version 3.0.0 line 84 column 79 error null


Also I'm going to upload a copy of the spreadsheet we got back. I'm not sure how Apple expects me to be able to correct these errors because they don't always specify where to find them in the file. They'll say "line x" but I don't know which html file it is referring to.

1) If you run epubcheck on the EPUB file you attached, you get:

$ epubcheck 9781621374008_EPUB.epub 
Epubcheck Version 3.0.1

Validating against EPUB version 2.0
ERROR: 9781621374008_EPUB.epub/OEBPS/content.opf(84,79): 'a4': fragment identifier is not defined in 'OEBPS/Text/toc.html'

Check finished with warnings or errors

You need to check your content.opf file, which is referring anchor 'a4', which indeed is not present in toc.html.

2) The spreadsheet you linked contains indeed only 1 line referring to EPUB file 9781621374008_EPUB.epub --- all the other lines seem related to other EPUB files.

3) Complain with Apple for their poor error reporting.
