This gets to a question I have had. When a book is converted from a Word .doc to epub or any other e-book format, does the table of contents index automatically generate ONLY if there is a numbered chapter heading? (I know I could use OpenOffice or Sigil, but I'm interested in the answer as it applies to Word. )

It depends on how you have the Word Doc formated. I use "Heading I" for all the chapter headings. When I do that Smashwords, Amazon and Calibre all seem to know to create the TOC. If I just bold/center or whatever the chapter headers, they do not create a TOC.
