When I read Mobi books in the Cybook, they always opened at the cover. At least with my own .mobi files, created with MobiPerl, with no "firstpage" and with an off-flow cover picture.

Yes, this seems to be a peculiarity of the Gen3 that it does that. Whether it opens at the cover or the first page, though, is a difference of one page forward or page back, so it makes no practical difference.

Add the cover as the first item in the "Manifest" section of the OPF file.

There certainly should be one: it's the section of the file which lists the files that make up the book, and the order in which they appear. If you open the OPF in Mobipocket Creator, the manifest is the list that appears on the "Publication Files" screen.

The Mobi file spec has not changed, so no, it's still done the same way.
