Most eBooks created in the last few years are generated originally as HTML, and hence will have full formatting. Old ones, however, tended to be simple ASCII files, and will probably have lost much of their original formatting as a result.

I try to restore italics and diacritics in the books I produce, if possible (and read them before uploading). That means comparing with paper editions or scans if they are available. I did that with "The Forsyte Saga", for instance, where the original Project Gutenberg text had many of these things missing. Other producers would probably not take so much trouble. Regardless of what the book producer does, you cannot know the quality of the source text and formatting without checking.

Proofing a book against a printed edition or an on-line scan is a massive undertaking; I generally reckon on taking a month to properly proof a good-sized book. I'm trying to do that for my favourite books, but it's a long, slow process.

Yep - I've been working on one of my favourite books for a couple of weeks in my spare time and I reckon I've got at least another week or so in order to get it right (I'm only spending the time because it's one of my favourite books ever).
Book is about 350 pages of pbook.

When I say working on, I mean comparing to original printed text to spot quotations, italics, paragraph ends etc etc..
