Creating a table of contents

I have tried to go though the instructions on doing so but I am having issues. This was ran through Calibre once and now I want to see if I can create a table of contents. The tutorial mentions <h1>, <h2>, etc. but mine below is where it has CHAPTER all the time and the chapter number like 1.

Can someone help me out here?

If that's the actual code in your book I would say your book is basically corrupted. All I can suggest is trying Heuristics with it's markup chapters option - though that will fail if your code actually looks exactly like that. Try it on the original version of the book, not one you've already converted a few times.
