Creating a table of contents

I have tried to go though the instructions on doing so but I am having issues. This was ran through Calibre once and now I want to see if I can create a table of contents. The tutorial mentions <h1>, <h2>, etc. but mine below is </p> where it has CHAPTER all the time and the chapter number like </p>1.

Can someone help me out here?

If that's the actual code in your book I would say your book is basically corrupted. All I can suggest is trying Heuristics with it's markup chapters option - though that will fail if your code actually looks exactly like that. Try it on the original version of the book, not one you've already converted a few times.
