For those running Windows, Avery Labels has absolutely FREE software for you to create your own custom DVD inserts - If you want card stock for a DVD insert, your local office supplies store should carry Avery® Matte White DVD Case Inserts for Inkjet Printers 8891, Pack of 20.

Windows DVD template
Here's simple one. You can do as I did, just pull out the original insert, print on its white side, and re-insert Hope the attachment comes through.
