I did have similar issue with the book covers on the SD Card.
You may want to give this try:
- Plug your Kobo to a PC
- Go to your SD Card
- Go to koboExtStorage->images
- Cut out all the files in the images direct and paste on your PC
- Unplug the Kobo
- Turn off/on the Kobo
- This would force the Kobo to regenerate all the book covers again
- If everything turns out okay, you can delete the files from your PC
