Basically, epub to mobi via callibre gets you an in-line TOC, but strips out some of the <guide> information that mobi readers would otherwise use. OTOH kindlegen and mobipocket publisher ignore the toc.ncx file from an epub, so don't build an inline toc. There's a way to have both, but it's a little involved. Details in the other thread.
