With a windows machine Try :-
Turn off the PC (Also worth unplugging the computer and holding the on button on the computer for about 5 seconds to clear any static charge)
Plug in the Kobo
Turn on the PC
Log on to windows

The bios / boot up may fix the USB connection (I used to loose USB connectivity on ocassion while hacking my JetBook and found this fix by accident when leaving it plugged in once)
