ADE reads PDF but builds its TOC similar to ePub so there is something in the way the TOC data is configured that is causing the problem. Perhaps a simpler TOC and then try variations to find out what it is that ADE doesn't like but it is likely due to TOC entries having multiple lines or some sort of formatting in them.
