The "Calibre icon page" is a book cover generated by Calibre if there is no proper cover available, either in the book or in the metadata, during conversion. The presence of that cover would indicate that you converted at least once without having a defined cover.
The fact that there is a cover in the books detail pane would indicate that you later defined a cover in the metadata. Calibre would try to embed the cover when you export the book from the library, that would, however, necessitate that you use either the save to disk or the send to device- feature in the GUI. If you manually copy the book from its folder, Calibre won't know about that and consequently can't update the cover.
Sometimes Calibre fails to embed the cover on export for some reason, in those cases, try a conversion to the same format (ePub->ePub in your case) to embed the cover in the file.
