How to generate cover from the first page of an ebook?

So I've looked through the documentation and am stumped .....I understand

1. How to generate a default (or custom using the plugin) cover from metadata.

2. How to download a cover for a book on one of the sites with book info.

3. How to add a cover from a jpeg or other discrete image.

But I'm trying to do something else. At times when I add a file, it seems more often with PDFs, Calibre will show the first page of the document as the cover. I'd like to be able to do this manually, and also to set it as the default behavior when books are added. Is this possible? How do I do it? I'm using Calibre for PC, and I read books on a Kindle 3/Keyboard.
