Port for QT5
I have ported the Barnes & Noble plugin for Qt5.

Remove the old plugin and install this one.

Tested with calibre 1.202.0 and calibre 1.44 using linux Mint 17.

Updated the plugin with comment lines describing the changes for QT5 and updated the changelog.txt file listing the test version.
