We're still using LyX (
http://lyx.org ) on Ubuntu here for our writing, for pBooks it's just too easy. For eBooks I dump to HTML and then fine tune it in vim.
For covers, I mostly Inkscape (
http://inkscape.org ) due to the great quality but infrequently require GIMP to do some image touch-ups.
