If you want to create pdf files with clickable TOC. Here is some of my experience. FYI.

Prerequisite:
MS WORD: I don't know if other text editors will do.
ADOBE ACROBAT (not Acrobat Reader): it will install some plugin in MS WORD for user to create pdf file with clickable TOC (That's why I have to stick to MS Word). I would suggest to update Acrobat to version 8.12 or above. PDF utilities other than Acrobat is beyond my knowledge.

Steps:
1. Make TOC in MS Word.
2. Create pdf in MS Word using plugins provided by Acrobat.
3. It will take much more time to build pdf with clickable TOC than un-clickable TOC. Go out for a walk and let your machine do its job.

Add-on:
I just found that OpenOffice alone can export PDF with clickable TOC. Pdf utilitiy is not required. Maybe you should try it. OpenOffice supports vertical text direction also.
