Ah... that tutorial is very old. The sgf option went away (before I ever started using Sigil, apparently
).
As far as the image question: are you sure you saved the epub and ran the validation? The warning you're getting would make sense if the image was in the "Images" folder and marked with the "Cover Image" semantic but not yet used in any (x)html file (<img alt="" src="../Images/cover.jpg" />). But if you say you inserted the image into the book... that warning shouldn't be occurring.
Regardless... most people have a separate (x)html file (the very first in the list in Sigil's Book Browser) that only contains the cover image and nothing else for dealing with cover images. This page is should also be marked with the "Cover Image" semantic.
