Missing TOC, top/bottom margins and hyperlinks in ePub -> PDF conversion

I created an ePub ebook with Sigil and I am trying to convert it to A4 PDF with Calibre 0.7.57 under Linux, but there are a few formatting issues. Although the ePub file includes a Table of Contents in the metadata, the TOC is missing from the PDF output

Also, the top and bottom PDF page margins are apparently ignored and don't change from the defaults, regardless of which values I set in the Top and Bottom fields of the conversion dialog under Page Setup > Margins.

Finally, although the text in the ePub file contains internal links to bibliographic entries in a References section, they are formatted in the PDF output but are not active: clicking them does nothing. External links to web sites are instead correctly preserved and are active.

Converting to PDF with Calibre another similar ePub ebook I also created with Sigil, there are the same missing TOC and top/bottom margins issues, but in addition the text in the title page, although only a few lines long and much shorter than an A4 page, is split between two pages of the PDF output.

Any troubleshooting suggestions or fixes?
