Read the sticky post I linked to. What you describe is a problem with the underlying program calibre uses for PDF conversion. If I recall correctly to get around this problem some folks use Mobipocket Creator (a Windows program) to convert the PDF to html or mobi (prc) and then add the converted file to calibre to convert to ePub.
