Never been able to nail down the specific steps to make it happen (I think I've developed unconscious workarounds), but Sigil definitely makes it happen under certain conditions. |

I think the Fragments come from splitting a book that used relative links (valid until the split happens)
Another possible case is deleting a file that has links pointing to it, then splitting that
