Eine Teillösung ist nun doch gefunden, allerdings ohne das Problem wirklich zu kennen. Die von Jutoh erzeugte ePub habe ich in Calibre erneut nach ePub konvertiert. Nun funktioniert innerhalb einer TOC-Ebene das Springen von Kapitel zu Kapitel. Immerhin!
