@HarryT: The metadata cover in a MOBI file is not part of the normal book contents, so you cannot point to it in the OPF guide.
If you include a jpg of your cover as the first page of your document, you can define it as the first item in your table of contents. When the Kindle opens your book, it should go to the first item in the TOC.

At least, that's how it worked for me.
