In Preferences | Add/Save | Adding books tab
"Read metadata only from file name" is un-checked.
In Saving books tab, these are checked:
"Save cover spearately"
"Update metadata in saved copies"
"Save metadata on OPF file"
I suspect the OPF file is the critical one here.
