Not HTML is your first problem.
Until that is corrected, the Combining HTML feature won't work (AFAIK that is the only format Calibre can combine).
You should either use an authoring program made for
the format
or
convert (and debug-edit) each single document to HTML, then make your index.html in Notepad (or any text editor)
(note the Index
must be able to access
all the files from where it resides when you add it to Calibre.
