I'm guessing it's simply an oversight of the ebook creation process for that particular title. The Kindle for Mac app version of the book (where you mention the ToC does work) is capable of reading Amazon's latest KF8 format—so that's what it downloads if it's available. The Kindle for iOS app doesn't yet deal with the newer KF8 format and so it downloads the regular old MOBI format. Apparently, the MOBI-only format of that book has no NCX ToC or inline HTML ToC to display.

The easiest way to ensure your calibre files have a ToC is to start with one of the KF8 versions (AZW3; and DRM-free of course) that has a working ToC to begin with.
