When the first (main) GUI screen has the 'More Options...' box checked (as in your screenshot), it should allow the next (options) screen to be visible and usable. You should see the below screenshot.
In your case, it is not.
What system is the GUI running on? Windows XP? Vista? under wine?
Did you install the GuteBook software in the default location i.e. C:\Program Files\GuteBook?
Try replacing the 'C:\Program Files\GuteBook\GuteBook1.ini' and 'C:\Program Files\GuteBook\GuteBook2.ini' with the ones in the below fresh set (see .zip).
