Does calibre show the connected iDevice? If so, you should be good to go.

Following the above steps didn't work, so we restarted Calibre in debug mode and saw that although the iphone had been booted off in itunes, Calibre was still reporting that there were two connected devices. Turning all the apps off (itunes and calibre) and shutting down the phone entirely, then restarting Calibre so that it restarted itunes itself with the phone not visible, made the ipad work.

Turning the phone back on, then turning wi-fi off, the ipad wasn't detected again (?!). At this point we restarted Calibre in debug mode to be absolutely sure it wasn't seeing the phone, and ..... the ipad was detected again. After that, the next normal opening worked again.

So far, this is the set of conditions that do work:
Itunes running already.
The ipad on, connected and visible in itunes.

The phone off or with wi-fi disconnected - just ejecting it from itunes is not enough, it has to actually be off the network to disappear, and it seems to take ~3-5 minutes to really disappear, or calibre just insists it's still there anyway. Also if Itunes has auto-ejected the ipad, nothing at all works until you disconnect and reconnect the cable. I suspect this may bite other people, it's not that uncommon to have multiple apple devices hanging around the house!

In any case, thank you very much for the help, it did point us in the right direction, and while it's still a little sensitive, thank you also for the plugin, it's working pretty great for a version 0.0.4, once we got it there.
