That is what I meant by the "TOC tracking bug". If open a book and read it through several chapters, the current chapter is not updated. If you open the TOC, it thinks you are on the first chapter. The navigation bar starts with the same chapter.

I hadn't noticed that before. But, it fits with the not tacking the chapter problem.
