Imagemagick ist immer wieder einen Blick wert:

convert -density 600 -contrast -gamma 0.1 -colorspace GRAY input.pdf output.pdf
Dabei verliert man allerdings den "content" und die Dateigröße kann sich stark ändern. Vielleicht hilfts ja beim einen oder andern Dokument.
