I've been trying to convert this to a format the Reader could easily display:

But I couldn't get acceptable results, could a Reader wizard please give it a shot and try to convert it to something readable on the Reader?

I converted it to:
*.doc and *.html

i think you can get the best result with the html- file and BookDesigner! - You sure have to do some work manually, but i think its a good start!

you'r right, but as i said, you can use the html with book designer, or you could do an rtf out of the word- file...
there is always work to do after convertions, but i think with the attached file, you have a good start.
